Why Do Some Wines Give You a Headache?

If you feel a throbbing headache coming on after a glass of wine, you're not alone: ​​it happens to many more people than you think, especially with red wine, and it particularly affects those who already suffer from migraines.

However, it's not an automatic or universal reaction: some people feel pain after just a few sips, while others, drinking the same amount, have no problems at all. This is the first indication that wine headaches aren't caused by a single culprit, but by a combination of factors involving the body, brain, and drinking context.

Alcohol, Dehydration And an Empty Stomach

Before wondering about sulfites or histamine, which are often blamed, it's worth starting with what wine does to the body. In many cases, headaches aren't caused by a "mysterious" substance, but by three basic physiological mechanisms that come into play every time you drink, often without you realizing it.

Alcohol and The Brain's Vascular Response

When you drink wine, the alcohol doesn't stay confined to your stomach. Within minutes, it enters your bloodstream and causes blood vessels to dilate, including those in the brain. In those predisposed, this dilation activates pain receptors and stimulates the release of CGRP, a molecule already well-known to migraine sufferers. The result can be a throbbing headache that occurs even with moderate amounts, especially if consumed quickly or in a short period of time.

Dehydration and Electrolyte Balance

Wine has a diuretic effect that is often underestimated. When you drink, you lose fluids faster than you think, and if you don't replenish your water intake, your body enters a state of mild dehydration, which is sufficient to cause problems. In the brain, even small electrolyte imbalances make the nervous system more reactive and less tolerant to stimuli, lowering the pain threshold. This is one of the reasons headaches can appear or worsen during the evening.

Why Drinking on An Empty Stomach Makes Everything Worse

Drinking on an empty stomach deprives the body of one of its main defenses. Without food, alcohol is absorbed much more quickly, peaking in the bloodstream sooner, and the liver is forced to metabolize it more abruptly. Under these conditions, a headache can appear within 15-30 minutes, even after just one drink. Eating before drinking is therefore not just general common sense advice, but a real prevention tool.

Sulfites, Histamine and Flavonoids: What Has Really Been Clarified?

When discussing wine headaches, attention often focuses on individual substances. In reality, research shows that there is no single culprit: some substances have long been overestimated, while others are only now emerging as more convincing possible explanations.

Are Sulfites the Sole Responsibles?

Sulfites are preservatives, present naturally in wine or added in small quantities, that serve to protect it from oxidation and keep it stable over time. For years, they have been identified as the main cause of headaches, but the data tell a different story.

White wines contain, on average, more sulfites than red wines, yet they are much less frequently associated with headaches. Furthermore, the amount present in a glass of wine is negligible compared to what the body is able to metabolize on a daily basis. Therefore, for the vast majority of people, sulfites are not the determining factor.

Histamine Are a Valid Explanation for Only a Few

Another frequently used name is histamine. It's a biogenic amine, a substance naturally formed during fermentation processes and involved in inflammatory responses in the body. In a small portion of the population, about 8-10 people out of 100, histamine is more difficult to break down. Even in these cases, however, studies show that histamine levels in wine are similar between those who develop headaches and those who don't, suggesting it's not a primary cause, but at most a secondary factor.

Quercetin, The Flavonol That Can Make a Difference

More recent research has instead shifted attention to flavonols, a family of natural compounds belonging to the polyphenol family, found primarily in grape skins. Among these, quercetin has attracted the most interest. During alcohol metabolism, quercetin is transformed into a molecule that inhibits the ALDH2 enzyme, which is needed to eliminate acetaldehyde, a toxic substance produced by the body when it digests alcohol. If the enzyme slows down, acetaldehyde builds up and can promote inflammation, redness, and headaches. This mechanism helps explain why red wines, which are richer in flavonols, are more often implicated than white wines, which contain about ten times less.

Alongside these compounds, tannins and other biogenic amines, produced primarily during malolactic fermentation, also come into play. They may contribute to the condition in predisposed individuals, but there is no direct and universal relationship with headache. This is also why wines with few sulfites are not automatically more tolerable: reducing one factor does not eliminate the entire mechanism.

Genetics, Practical Choices and When It's Best to Avoid Wine

The response to wine is also a genetic issue. Approximately 40% of the East Asian population possesses a less efficient variant of the ALDH2 enzyme , which leads to acetaldehyde accumulation even with small amounts of alcohol. In these cases, headaches, flushing, and nausea are not a random side effect, but a predictable biological response.

To reduce the risk, the most effective strategy is behavioral: drinking water between drinks, avoiding drinking on an empty stomach, and slowing down helps reduce the impact of alcohol. Wine choice also matters: whites, rosés, and light reds are generally more tolerable. If you prefer red, Pinot Noir is known to have lower histamine levels than other varieties.

If wine is a constant and intense problem for you, avoiding it is a clear choice. If the problem is sporadic, however, understanding how your body reacts allows you to make better choices, drink more consciously, and let wine remain what it should be: a pleasure, not a problem.
